1. Signed application letter specifying the Position Title, Item Number, and Office applied for;
2. Fully accomplished and signed Personal Data Sheet (PDS) and Work Experience Sheet (WES) using CS Form No. 212. PDS must include a recent passport-size picture and three (3) Character References (either a former supervisor/ professor/ anyone superior who has overseen the applicant's work and interpersonal abilities). WES can be downloaded at www.csc.gov.ph or www.denr.gov.ph. Include all relevant work experiences in the WES;
3. Proof of eligibility such as:
     a.) CSC certificate of eligibility
     b.) PRC board rating and updated license
     c.) If BAR passer: Supreme Court of the Philippines-issued Certificate of Membership in the Philippine Bar
4. Authenticated copy of Transcript of Records (TOR) and diploma for Bachelor’s degree and postgraduate degree (for SG 24 and above); Authenticated copy of Transcript of Records (TOR) for Bachelor’s degree and postgraduate degree (for below SG 24); Submit all that applies: elementary diploma, HS diploma, Bachelor’s, Master’s, Doctorate, Diploma course, Certificate course, if ongoing or not completed: True copy of grades showing number of units earned);
5. HR - certified copy of performance rating sheet in the latest rating period (JANUARY - JUNE 2024); as applicable
6. Certified photocopies of Training Certificates;

8. Certificate of Employment / Contract of Service / Memorandum of Agreement from all previous employers.
